Care Manager I - Social Work (MSW/LCSW) PD : Irvine Care Management
Job Description
The Care Manager (LCSW/MSW) coordinates patients’ needs through the continuum of care, encompassing pre‑admission through post‑discharge plans. This role collaborates with physicians, nurses, clinical staff, and community agencies to identify and arrange appropriate care.
Reviews clinician assessments and patients’ financial, family, and psychosocial support to develop comprehensive care and/or discharge plans. May focus on specific aspects of Care Management such as discharge planning, utilization review, or psychosocial support. May review records to assess appropriate admission status, level of care, payer source, and UR contracts to validate billing. Provides psychodynamic intervention and crisis counseling to support patients and families. Educates patients and families about healthcare options and connects them with resources.
Documents pertinent patient issues, contacts, and plans in the medical record. Is a mandated reporter for elder, child, and spousal abuse. The Community Care role specifically supports high‑risk, homeless, and mental health populations. Performs other duties as assigned.
Community Health : Provides care in outpatient and community settings. Coordinates and provides safe, timely, effective, equitable, and client‑centered care. Manages client needs through intake to case termination plans, assesses needs, develops tailored plans, records progress, collaborates with partners, and provides crisis counseling as needed. Facilitates psychoeducational groups and assists in training interns.
Qualifications
Master’s degree in Social Work (MSW) required.
One year of MSW experience in an acute healthcare setting preferred.
One year of MSW experience in community case management preferred.
Licensed driver with an insured automobile in good working order required.
Bilingual or multilingual preferred.
CA Driver’s License.
