Overview The Care Manager/"Designated Care Manager" is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long term care and reminiscence neighborhoods. The Care Manager provides hands on care and physical and emotional support as outlined in each resident's Individualized Service Plan (ISP) while maintaining a safe and comfortable home-like environment. The Care Manager/"Designated Care Manager" demonstrates Sunrise Senior Living's Mission, "to champion quality of life for all seniors" in accordance with laws, Sunrise standards, policies, and procedures to promote the highest degree of quality care and services to our residents.
Responsibilities Resident Care: Review, read, notate, and initial Daily Log to document and learn about pertinent information about residents; participate in the development of ISPs and monthly updates; review designated assignments; manage a designated group of residents during the shift and physically check on them; observe, report, and document symptoms and conditions; attend daily Cross Over meetings; notify supervisor of increased care needs; greet guests and residents; respond to calls in a timely, courteous manner; assist with continence management and sanitation; communicate with families; promote independence and dignity; assist with ADLs and life skills; engage residents in reminiscence and activities; maintain a warm, safe, and clean environment; adhere to safety regulations and infection control practices.
Risk Management and Safety: Ensure compliance with health and safety regulations; use PPE and safety equipment; report accidents and hazards immediately; maintain clear pathways and safe storage of oxygen and other hazardous materials; understand exposure protocols for bloodborne pathogens and hazardous chemicals.
Housekeeping and Dining: Maintain resident rooms and common areas; manage laundry; assist with dining room service, meal preparation, and dietary needs per ISP; observe dining changes and maintain Sunrise hospitality standards.
Reminiscence Neighborhood Responsibilities: Lead IEA activities for residents in life skills and enrichment, facilitate monthly letters with families, and create pleasant daily experiences for residents.
Training and Team Contribution: Participate in team goals, uphold Team Member Credo and Sunrise Principles of Service; engage in engagement programs and trainings; attend meetings and complete required training; comply with infection control and regulatory requirements; perform other duties as assigned.
Qualifications High School diploma/GED (as required by state/provincial regulations)
CPR and First Aid as required by regulations
Must be at least 18 years of age
Previous experience with seniors preferred
Ability to make decisions in the resident's best interest and communicate effectively
Drivers who drive a Sunrise vehicle must sign the Driver job description where applicable
