Description Of Work
The primary purpose of the Care Manager is to ensure that the individuals served are provided a comprehensive array of training, services, and supports that enables them to achieve maximum independence in self‑care and activities of daily living; including gaining and maintaining significant relationships, self‑management and ensuring the protection of their rights.
The Care Manager establishes, implements, and monitors program goals, quality controls, and policies/procedures for defined residential units or homes. Additionally, the Care Manager evaluates, modifies, and implements individualized cognitive, vocational, and/or social developmental care plans. The Care Manager is responsible for auditing annual training of staff within the home.
The Care Manager working hours will be Monday – Friday from 8:00 a.m. – 5:00 p.m.
Knowledge, Skills, and Abilities / Management Preferences
Thorough knowledge of concepts, practices, and techniques of the field of developmental disabilities and the population served to include evaluations and development of comprehensive life plans.
Skill in writing life plans including punctuation, grammar, etc., and reports to document individual treatment plans; Skill is needed in auditing records and programming activities.
Excellent communication skills to effectively work with people and the ability to recognize and resolve problems.
Current knowledge of the standards, regulations and licensure requirements.
The ability to coordinate the activities of a group of professionals of varied disciplines; to provide leadership in the development, implementation and evaluation of treatment plans; and to develop cooperative working relationships with a variety of persons providing services to the individuals.
Salary Range
$35,984.00 – $62,971.00
Minimum Education and Experience
Bachelor's degree in a human services or related field from an appropriately accredited institution, and one year of experience providing and/or developing person‑centered care plans for individuals with intellectual disabilities/co‑occurring disorders; or an equivalent combination of education and experience. Advanced degrees in human services may be substituted to meet educational requirements if an applicant does not possess a Bachelor’s degree in human services or a related field.
