Job Overview
The Care Manager / “Designated Care Manager” is responsible for providing high‑quality care and services to a consistent group of residents and their families in our assisted living and long‑term care neighborhoods. The Care Manager provides hands‑on care, physical and emotional support and maintains a safe, comfortable home‑like environment in accordance with federal, state, provincial, and local laws and Sunrise Senior Living standards.
Responsibilities
Review and document daily logs, provide updates and maintain resident records.
Participate in development and monthly updates of Individualized Service Plans (ISPs).
Oversee assigned residents during shifts, including safety checks and symptom monitoring.
Respond promptly to resident care needs, call bells, and security alerts.
Assist with continence management, bed care, and daily living activities (ADLs).
Maintain cleanliness and hygiene standards; practice standard care precautions.
Serve meals and promote positive dining experience; adhere to dietary plans.
Lead resident activities, including life‑skills, reminiscence, and social events.
Engage in risk management, safety procedures, and emergency response.
Participate in team meetings, training sessions, and orientation.
Support training and development of staff; contribute to engagement initiatives.
Maintain accurate documentation for family and regulatory reporting.
Fulfill additional duties as required by supervision.
Qualifications & Skills
High School diploma or GED (preferred).
CPR Certificate; First Aid certification (state required).
Minimum 18 years of age.
Previous experience working with seniors preferred.
Strong communication, empathy, and interpersonal skills.
Ability to work independently and as part of a team.
Awareness of resident rights and care standards.
