Overview Care Manager - Behavioral Health - Family Medicine Center at McLeod Health. This role focuses on the coordination and delivery of comprehensive psychiatric care at MBHS.
Responsibilities Responsible for the overall coordination and delivery of comprehensive psychiatric care at MBHS
Conduct psychosocial assessments upon admission
Maintain communication with patients and their families to assess baseline functioning and coordinate needed resources
Serve as a group leader for process-oriented and psychoeducational groups for psychiatric inpatients as needed
Actively participate in Multidisciplinary Treatment Team and provide pertinent and relevant information
Facilitate and coordinate psychiatric commitments by serving as a liaison between the probate court and MBHS
Perform all other duties as assigned
Requirements Masters Degree
