Overview The Care Manager/"Designated Care Manager" is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long term care and reminiscence neighborhoods. The Care Manager provides hands on care and physical and emotional support as outlined in each resident's Individualized Service Plan (ISP) while maintaining a safe and comfortable home like environment. The Care Manager demonstrates the Sunrise Mission, "to champion quality of life for all seniors" in accordance with laws, Sunrise standards, policies and procedures.
Responsibilities Resident Care: document and learn about residents via Daily Log; develop ISP updates; monitor designated residents; observe and report changes in condition; participate in Cross Over meetings; notify supervisors of increased care needs; respond to call bells and maintain safety; greet guests, answer phones; assist with continence management; communicate with families; support independence and safety; assist with ADLs and life enrichment activities; engage residents in reminiscence; maintain warmth and dignity; follow safety regulations and infection control; host and engage in daily activities.
Risk Management and General Safety: ensure compliance with health and safety regulations; use PPE and safety equipment; report accidents/incidents and unsafe conditions; keep hazards clear; handle potentially infectious materials safely.
Housekeeping and Laundry: maintain and clean resident rooms; launder resident clothing; manage dining room linens; complete housekeeping tasks.
Dining Service: serve meals, maintain a pleasant dining experience, assist with setup/cleanup, follow diets, document dining changes, provide room service as needed; safe food handling.
Reminiscence Neighborhood Care: support residents in life skills and daily routines; plan and participate in social and enriching activities; integrate resident life skills into daily schedule; employ multi-sensory experiences; engage families with monthly letters.
Training and Team Contributing: participate in team goals; uphold Team Member Credo and Sunrise Principles; engage in engagement programs and surveys; attend meetings and trainings; follow infection control policies; maintain required trainings; perform other duties as assigned.
Core Competencies: remain calm under pressure; manage multiple priorities; effective written and verbal communication; organizational and time management; good judgment and problem solving.
Qualifications High School diploma/GED (as required by state/provincial regulations).
CPR and First Aid as required by regulations.
Must be at least 18 years old.
Previous experience with seniors preferred; desire to serve and care for seniors.
Ability to act in the resident’s best interest; if applicable, drivers sign the Driver job description for safety and regulatory compliance.
