Job Overview The Care Manager/“Designated Care Manager” is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long term care and reminiscence neighborhoods. The Care Manager provides hands on care and physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP) while maintaining a safe and comfortable home like environment. The Care Manager demonstrates the Sunrise Mission, “to champion quality of life for all seniors” in accordance with laws, Sunrise standards, policies, and procedures to promote the highest degree of quality care and services.
Essential Duties RESPONSIBILITIES & QUALIFICATIONS
As part of the Sunrise team, supporting our Mission, Principles of Service and Core Values is fundamental. The role emphasizes the sacred value of human life with responsibilities including the following sections:
Resident Care Review, read, notate, and initial Daily Log to document and learn about pertinent information about residents.
Participate in the development of the ISPs and monthly updates.
Review designated assignments.
Be responsible for a designated group of residents during the shift, know where residents are, and physically check on them throughout the shift.
Observe, report, and document symptoms and conditions of residents for changes in condition such as skin, behavior, alertness, weight, diet, and participation in activities.
Attend daily Cross Over meetings by the lead care manager.
Notify supervisor and resident care director if a resident has increased care needs.
Inform supervisor of any resident changes in condition.
Respond to security system and resident call bells promptly and take appropriate action including resetting call bells.
Greet guests, family members, residents, and team members; answer, direct and respond to calls in a timely, courteous, and professional manner.
Assist with continence management and dispose of continence products properly to ensure sanitation.
Communicate with families and serve as a resource as needed.
Help residents maintain independence and physical safety and promote dignity in line with Resident Rights and Sunrise Principles of Service.
Participate/lead and assist residents with ADLs and Invite, Encourage, and Assist (IEA) per ISP.
Engage residents in life skills and other life enrichment activities; promote reminiscence activities.
Understand and respond to each resident with empathy considering unique communication patterns or history and basic human needs.
Maintain a warm, personal, and calm environment while following safety regulations and Standard Care Precautions.
Host and engage in activities with residents daily.
Risk Management and General Safety Partner with the community team to ensure compliance with occupational health and safety regulations; promote Risk Management programs and policies.
Practice safety procedures at all times including PPE, fire safety equipment, SDS, and Lockout Tagout where applicable.
Report accidents/incidents and unsafe conditions immediately.
Keep cords, carts, equipment, and hazards out of walkways; comply with fire codes.
Follow proper procedures for exposure to blood borne pathogens, bodily fluids, infectious waste, and hazardous materials; report occupational exposures immediately.
Store and monitor oxygen tanks safely and according to prescriptions.
Housekeeping and Laundry Services Maintain and clean resident rooms, including beds, trash, clothing organization, and floor care.
Wash resident laundry as ISP notes require; manage dining linens as needed.
Complete assigned housekeeping tasks; keep common areas clean and tidy.
Dining Service Serve meals in the dining room and contribute to a pleasant dining experience.
Assist with dining room setup/cleanup; participate in pre-meal meetings.
Follow residents’ diets per ISP and Confidential Diet Board/Chart; document dining habit changes.
Respond to dining needs while upholding Sunrise hospitality and service standards; provide room service as needed.
Practice safe food handling in line with universal precautions.
Specific Responsibilities for the Reminiscence Neighborhood Care Manager IEA residents in life skills and enrichment activities per ISP and demographic profile.
Encourage attendance at afternoon social and integrate life skills into daily routines.
Incorporate multi-sensory experiences and “Creating Pleasant Days” concepts into daily care.
Engage in family communications through monthly letters and interactions.
Training and Team Contribution Work as part of a team; contribute to team goals and member credo; support sunrise Principles of Service.
Support engagement programs and participate in surveys and improvement workshops.
Attend regular meetings and training modules; comply with infection control and safety policies.
Maintain required trainings and adapt to regulatory requirements; perform other duties as assigned.
Core Competencies Ability to stay calm in difficult situations; multitask effectively.
Strong written and verbal communication; good organizational and time-management skills.
Good judgment, problem solving and decision-making abilities.
Experience and Qualifications High School diploma/GED (as required by regulations).
CPR and First Aid as required by regulations.
Must be at least 18 years old.
Previous experience with seniors preferred; desire to serve and care for seniors.
Ability to make decisions in the resident's best interest.
If applicable, drivers for Sunrise vehicles must sign Driver job description and safety duties.
