Job Description The Care Manager is responsible for providing the highest quality care and services to a consistent group of residents and their families in our assisted living/long term care and reminiscence neighborhoods. The Care Manager provides hands-on care and physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP) while maintaining a safe, home-like environment. This role demonstrates Sunrise’s Mission: to champion quality of life for all seniors, in accordance with laws, regulations, and Sunrise standards, policies, and procedures.
Responsibilities & Qualifications Essential Duties As part of the Sunrise team, support of our Mission, Principles of Service, and Core Values is fundamental. Core responsibilities include:
Resident Care Review, read, notate, and initial Daily Log to document and learn about residents.
Participate in the development of ISPs and monthly updates.
Be responsible for a designated group of residents during the shift; know resident locations and check on them throughout the shift.
Observe, report, and document symptoms and conditions for changes (skin, behavior, alertness, weight, diet, participation in activities).
Attend daily Cross Over meetings with the lead care manager.
Notify supervisor and resident care director if a resident has increased care needs.
Inform supervisor of changes in a resident’s condition.
Respond to security system and resident call bells promptly and appropriately; reset call bells when needed.
Greet guests, family members, residents, and team members; handle calls professionally.
Assist with continence management and proper disposal of products to ensure sanitation.
Communicate with families and act as a resource as needed.
Help residents maintain independence and safety while promoting dignity, adhering to Resident Rights and Sunrise principles.
Participate in/lead ADLs and Encourage/Assist residents to participate in ISP-directed activities.
Engage residents in life skills and enrichment activities, including reminiscence, throughout the day.
Demonstrate empathy and respect for each resident’s communication patterns and needs.
Maintain a warm, calm environment and follow established safety regulations.
Practice Standard Care Precautions for cleanliness, hygiene, and health.
Host and engage in daily resident activities.
Risk Management & General Safety Collaborate with the community team to ensure compliance with occupational health and safety requirements.
Follow safety procedures including PPE, fire safety, SDS, and lockout/tagout.
Report all accidents/incidents immediately and unsafe conditions.
Keep cords, carts, equipment, and hazards out of traffic and in compliance with fire codes.
Understand proper handling and disposal of potentially hazardous materials and exposures.
Report occupational exposures immediately.
Store and monitor oxygen tanks per safety guidelines.
Housekeeping & Laundry Maintain and clean resident rooms; manage laundry per ISP and routines.
Maintain common areas in a clean, tidy state.
Dining Service Serve meals and assist in dining rooms; promote a pleasant dining experience.
Assist with dining room setup and cleanup; participate in pre-meal meetings.
Follow residents’ diets as indicated in the ISP/Confidential Diet Board.
Document changes in dining habits in daily logs; respond to dining needs while upholding Sunrise hospitality and service standards.
Provide room service as needed; practice safe food handling.
Reminiscence Neighborhood IEA residents in life skills and enrichment activities as indicated on ISP.
Support residents to attend afternoon social events.
Incorporate the concept of “Creating Pleasant Days” into daily routines; integrate life skills into daily activities.
Training & Team Success Collaborate as a team; contribute to engagement programs and surveys.
Attend regular meetings and trainings; comply with infection control and regulatory requirements.
Maintain required trainings to meet Sunrise standards; perform other duties as assigned.
Core Competencies Ability to stay calm in difficult situations; manage multiple priorities.
Strong written and verbal communication; excellent organizational and time management skills.
Good judgment, problem solving, and decision-making skills.
Experience & Qualifications High School diploma/GED (as required by regulations).
CPR and First Aid as required by regulations.
Must be at least 18 years old.
Previous experience with seniors preferred; desire to serve and care for seniors.
Ability to act in the resident’s best interest.
