Overview Care Design New York is hiring a Care Manager. The role focuses on helping people with intellectual and/or developmental disabilities identify and realize their short- and long-term goals by developing, implementing, and monitoring person-centered Life Plans. The position emphasizes relational and organizational skills, training, and delivering high-quality service to members. The role is performed in a hybrid environment, including in-home visits, office trainings/meetings, and work-from-home duties. If supporting members outside New York City, a driver’s license, reliable transportation, and appropriate insurance are required.

Responsibilities Advocate for and with members to support informed decision making, informed consent, and appropriate guardianship.
Schedule, lead, and collaborate with members and an interdisciplinary team to develop a comprehensive, person-centered Life Plan aligned with the person’s needs and goals.
Implement, update, and monitor Life Plans and facilitate Life Plan reviews and approvals.
Coordinate integration of needed supports and services (medical, behavioral, social, habilitation, dental, psychosocial, community-based and facility-based long-term supports).
Maintain ongoing contact with key people in a member’s life.
Ensure timely submission of documentation (Life Plans, progress notes, etc.).
Assist members in maintaining benefits (Social Security, SSI, Medicaid, Medicare, and Food Stamps).
Help members resolve housing, utilities, judicial system, and general safety challenges.
Report abuse or neglect immediately when observed or reported.
Qualifications A Bachelor’s degree with 2+ years of relevant experience, OR
A License as a Registered Nurse with 2+ years of relevant experience, OR
A Master’s degree with 1+ year of relevant experience.
Strong verbal and written communication skills, with strong interpersonal and organizational abilities.
