Position Summary

The Care Manager is responsible for care management services of members enrolled in Florida Community Care (FCC) to promote effective education, self-management support, and timely healthcare delivery to achieve optimal quality and positive outcomes. The Care Manager demonstrates professionalism and promotes respect, honesty, care, and dignity for members and team members involved in FCC services. The role involves developing and maintaining individualized care plans using available data.
Essential Functions

Completes initial and annual assessments using the Florida 701B, all face-to-face, to identify conditions and resources.
Develops and reviews care plans with clients or their representatives.
Coordinates service delivery with other professionals and arranges appropriate services.
Provides education and training to caregivers, family members, or friends.
Maintains client records with progress reports and documentation.
Engages with members telephonically to maintain care plans using available data.
Maintains caseloads as per state requirements, including ratios for community and facility-based enrollees.
Evaluates and revises care plans to meet changing needs and goals.
Identifies community resources and makes appropriate referrals.
Promotes self-management and independence among members and caregivers.
Serves as a liaison for service coordination.
Provides education during care management activities.
Documents activities clearly and promptly in the electronic system.
Participates in orientation, supervision, team meetings, and in-services.
Performs other duties as assigned.
Education Requirements

Minimum of two years of relevant experience.
Bachelor’s degree in social work, sociology, psychology, gerontology, or related field.
Required Skills

701B certification preferred; all Care Managers must have it.
Must become certified in Person Centered Care Planning (PCCP).
Attend mandatory FCC training sessions as needed.
