Job Overview: The Care Manager/“Designated Care Manager” is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long term care and reminiscence neighborhoods. The Care Manager provides hands on care and physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP) while maintaining a safe and comfortable home-like environment. The Care Manager demonstrates Sunrise Senior Living’s Mission, “to champion quality of life for all seniors.”
Responsibilities Review, read, notate, and initial Daily Log to document and learn about pertinent information about residents.
Participate in the development of the ISPs and monthly updates.
Review designated assignments.
Responsible for a designated group of residents during the shift, know where residents are, and physically check on them throughout the shift.
Observe, report, and document symptoms and conditions of residents for changes in condition (e.g., skin, behavior, alertness, weight, diet, participation in activities).
Attend daily Cross Over meetings by the lead care manager.
Notify supervisor and resident care director if a resident has increased care needs.
Inform supervisor of any resident changes in condition.
Respond to security system and resident call bells promptly and immediately, taking appropriate action including resetting call bells.
Greet guests, family members, residents, and team members; answer and direct phone calls in a timely, courteous, and professional manner.
Assist with continence management and dispose of continence products properly to ensure sanitation of resident suites and community restrooms (using standard care procedures).
Communicate with families and serve as a resource as needed.
Help residents maintain independence and physical safety and promote dignity, adhering to the standards of Resident Rights and Sunrise Principles of Service.
Participate in/lead and assist residents with activities of daily living (ADLs) and Invite, Encourage, and Assist (IEA) residents to participate in activities as indicated on the ISP.
Engage residents in life skills and other life enrichment activities throughout the day in reminiscence.
Strive to understand and respond to each resident with empathy, mindful of the resident’s unique communication patterns or history and basic human needs.
Maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment.
Ensure established safety regulations are followed; practice good Standard Care Precautions of cleanliness, hygiene, and health.
Host and engage in activities with residents daily.
Risk Management and General Safety Partner with the community team to ensure compliance with national/provincial regulations, promoting risk management programs and policies, and adhering to safety rules and regulations.
Practice safety procedures at all times including PPE, fire extinguishers, SDS, and Lockout Tagout procedures.
Report all accidents/incidents immediately and report unsafe conditions/equipment.
Ensure hazards are kept out of walkways and exits are clear in compliance with fire codes.
Understand and practice proper methods for attending to and disposing of exposure to bloodborne pathogens, bodily fluids, infectious waste, sharp objects, and hazardous materials; report occupational exposures immediately.
Ensure oxygen tanks are stored safely and monitored to maintain prescribed levels.
Housekeeping, Laundry, and Dining Service Maintain and clean resident rooms, including making beds, trash removal, organization, and cleaning as needed.
Wash resident laundry as noted in the ISP, and wash/fold dining room linens and napkins; complete assigned housekeeping tasks and maintain common areas.
Serve meals in the dining room, promote a pleasant dining experience, assist with dining room setup/cleanup, follow residents’ diets per ISP, observe and document dietary changes, and provide room service as needed.
Reminiscence Neighborhood Care Manager Specific Responsibilities IEA residents in life skills and life enrichment activities as indicated on ISP and demographic profile.
Support afternoon social attendance and integrate life skills into daily routines with multi-sensory experiences.
Participate in monthly letters home and family communications.
Training and Team Contribution Participate as a team member and work toward team goals; demonstrate our Team Member Credo and Principles of Service.
Contribute to engagement programs and processes, participate in surveys and improvement planning, and attend regular meetings and trainings.
Comply with infection control techniques and training requirements to meet Sunrise standards; perform other duties as assigned.
Core Competencies Ability to remain calm in difficult situations; manage multiple priorities; effective written and verbal communication; strong organizational and time-management skills; good judgment and problem-solving abilities.
Experience and Qualifications High School diploma/GED (as required by regulations).
CPR and First Aid as required by regulations.
Must be at least 18 years of age.
Previous experience working with seniors preferred; desire to serve and care for seniors.
Ability to make decisions in the resident’s best interest; if applicable, sign driver duties for safety and regulatory compliance.
