Part-time Care Manager
All Care Wellness focuses on promoting physical, mental, emotional, and spiritual well-being. The practice blends traditional medicine with holistic therapies and may address anxiety, substance abuse, panic disorders, OCD, PTSD, ADHD, life skills, career, and relationship issues. Services include individual psychotherapy, medication management, home visits, and group therapy, in addition to Group Adult Foster Care Program services.
Role overview: The Care Manager provides support to members enrolled in the Group Adult Foster Care Program. As this is a home care program, travel to members\' homes is frequent. Reliable transportation is essential.
Care Manager Responsibilities
Completion of biopsychosocial assessments for each member
Bi-monthly home visits with each member
Maintain member records, charts, and other pertinent information
Participate in case conferencing
Provide direct member care as needed
Coordinate care with other treatment providers
Evaluate direct participant care performed by the Direct Care Aide
Develop, with the nurse, the member plan of care to be carried out by the Direct Care Aide
Collect and document members\' current health providers and make referrals as necessary
Collaborate within a multidisciplinary team including DCA, Nurse, and Program Coordinator
Respond to telephone inquiries related to member requests and referrals for services
Follow up on doctor\'s appointments, medical releases, and medical equipment
Essential Skills
Ability to assess, plan, implement, document, coordinate, and manage member care per facility standards
Strong communication and collaboration with members, families, physicians, and other health professionals
Ability to assess members for suitable community interventions
Ability to handle emergency situations in a timely and safe manner
Leadership to supervise Direct Care Aides
Proficiency in entering, accessing, and retrieving patient data and creating reports
Listening and attending skills, including clarifying and paraphrasing
Research and practice evaluation
Policy practice and effective use of supervision and consultation
Physical & Mental Demands
Good physical and mental health
Ability to travel to members\' homes within the service area
Mental fortitude to handle stress
Proof of negative TB screen
