Overview The Care Manager/“Designated Care Manager” is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long term care and reminiscence neighborhoods. The Care Manager provides hands on care and physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP) while maintaining a safe and comfortable home like environment. The Care Manager demonstrates the Sunrise Mission, “to champion quality of life for all seniors” in accordance with applicable laws, Sunrise standards, policies, and procedures.
Base pay range $18.50/hr - $21.20/hr
Responsibilities Resident Care: Review, read, notate, and initial Daily Log to document and learn about pertinent information about residents; participate in ISP development and updates; review designated assignments; manage a designated group of residents during the shift; observe, report, and document changes in residents’ condition; respond to security system and resident call bells; greet guests and residents; assist with continence management; communicate with families; help residents maintain independence and safety; participate in ADLs and life enrichment activities; engage residents with empathy and meaningful interaction; maintain a warm, safe, and clean environment; adhere to safety regulations and infection control practices.
Risk Management and General Safety: Ensure compliance with Occupational Health and Safety regulations; practice PPE and safety procedures; report accidents and unsafe conditions; maintain clear exits and proper handling of hazardous materials and bloodborne pathogens; safely manage oxygen equipment.
Housekeeping and Laundry Services: Maintain and clean resident rooms and common areas; handle laundry as ISP dictates; manage dining linens and dining room tasks.
Dining Service: Serve meals, maintain a pleasant dining experience, assist with dining room setup and cleanup, and follow dietary guidelines per ISP.
Reminiscence Neighborhood Care Manager responsibilities: lead life skills activities, support afternoon social programs, integrate residents’ life skills into daily routines, and provide multi-sensory experiences.
Training and Team Contribution: Participate as a team member, uphold Team Member Credo, contribute to engagement programs, attend meetings and trainings, and comply with infection control and regulatory requirements.
Core Competencies: Maintain composure in difficult situations, manage multiple priorities, communicate effectively, organize and manage time, exercise good judgment and problem solving.
Experience and Qualifications: High School diploma/GED; CPR and First Aid as required; minimum age 18; experience with seniors preferred; drive Sunrise vehicle as applicable and comply with safety requirements.
