Job Overview
The Care Manager/“Designated Care Manager” is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long‑term‑care and reminiscence neighborhoods. The Care Manager provides hands‑on care and physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP) while maintaining a safe and comfortable home‑like environment. This role champions the mission of Sunrise Senior Living: "to champion quality of life for all seniors" in accordance with federal, state/provincial, and local laws, regulations, and Sunrise standards.
Responsibilities & Qualifications
Resident Care
Review, read, notate, and initial Daily Log to document resident information.
Participate in development and monthly updates of ISPs.
Review designated assignments.
Manage a designated group of residents during the shift, ensuring their safety and well‑being.
Observe, report, and document changes in residents’ conditions (skin, behavior, weight, diet, activity participation).
Attend daily Cross Over meetings with the lead care manager.
Notify supervisor and resident care director if a resident’s care needs increase.
Respond promptly to security system alerts and resident call bells.
Greet guests, family members, residents, and team members courteously.
Answer, direct, and respond to phone calls promptly and professionally.
Assist with continence management and disposal of continence products, ensuring sanitation.
Communicate with families as needed.
Help residents maintain independence, physical safety, and dignity while adhering to Resident Rights and Sunrise Principles of Service.
Assist residents with ADLs and provide IEA (Invite, Encourage, Assist) toward activities indicated on ISP.
Engage residents in life‑skills and enrichment activities throughout the day.
Show empathy and respect resident communication patterns, needs, and basic human rights.
Maintain a warm, personal, and calm environment.
Follow established safety regulations and practice good Standard Care Precautions.
Host and engage in daily activity sessions with residents.
Risk Management and General Safety
Partner with community team to ensure compliance with occupational health and safety regulations.
Practice safety procedures, including PPE, fire extinguishers, SDS, and Lockout‑Tagout.
Report accidents/incidents and unsafe conditions promptly.
Maintain clear pathways, keep cords, carts, and hazards out of exits, and comply with fire codes.
Practice proper handling and disposal of bodily fluids and hazardous materials.
Report occupational exposures to blood, bodily fluids, and hazardous chemicals immediately.
Ensure oxygen tanks are stored safely and monitored.
Housekeeping and Laundry Services
Maintain and clean resident rooms, including bedding, trash, and laundry as needed.
Wash and fold linens and napkins.
Complete assigned housekeeping tasks and keep common areas tidy.
Dining Service
Serve meals in the dining room and assist with setup and clean‑up.
Promote a pleasant dining experience for all meals.
Participate in pre‑meal meetings and follow resident diets as per ISP and diet charts.
Document dining habit changes in the daily log.
Deliver room service as needed while upholding hospitality standards.
Use safe food handling practices in compliance with universal precautions.
Specific Responsibilities for the Reminiscence Neighborhood Care Manager
IEA residents in life‑skills and enriching activities per ISP.
Encourage residents to attend afternoon social sessions.
Integrate the concept of “Creating Pleasant Days” into daily routines.
Blend multi‑sensory experiences into resident activities.
Participate in monthly letters home and collaborative letter writing with residents and families.
Training and Contributing to Team Success
Commit to team goals and daily interactions guided by the Team Member Credo.
Participate in engagement program planning and surveys.
Attend regular meetings (Town Hall, Department, Cross Over) and training modules.
Maintain compliance with infection control, proper container placement, and removal techniques.
Complete all required training from state or regulatory authorities.
Perform any other duties as assigned.

Experience and Qualifications
High School diploma/GED as accepted or required per state regulations.
CPR Certificate and First Aid as required by state regulations.
Minimum age of 18 years.
Previous experience working with seniors preferred.
Desire to serve and care for seniors.
Ability to make decisions in the resident’s best interest.
