Job Overview The Care Manager/“Designated Care Manager” is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long term care and reminiscence neighborhoods. The Care Manager provides hands on care and physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP) while maintaining a safe and comfortable home like environment. The Care Manager demonstrates the Mission for Sunrise Senior Living, “to champion quality of life for all seniors” in accordance with federal, state/provincial, and local laws and regulations and Sunrise Senior Living standards, policies, and procedures to promote the highest degree of quality care and services to our residents.

Responsibilities & Qualifications
Essential Duties As a part of the Sunrise team, supporting our Mission, Principles of Service and Core Values is a fundamental part of this job. The following responsibilities are included, among others, to support resident care and daily operations:

Resident Care: Review, read, notate, and initial Daily Log to document and learn about pertinent information about residents.

Participate in the development of the ISPs and monthly updates.

Review designated assignments and be responsible for a designated group of residents during the shift, knowing where residents are and checking on them throughout the shift.

Observe, report, and document symptoms and conditions of residents for changes in condition (skin, behavior, alertness, weight, diet, activities).

Attend daily Cross Over meetings by the lead care manager; notify supervisor of increased care needs or changes in condition; respond to security system and resident call bells promptly; greet guests, family members, residents, and team members.

Assist with continence management and disposal of products to ensure sanitation of resident suites and restrooms.

Communicate with families as needed; help residents maintain independence and safety while adhering to Resident Rights and Sunrise Principles of Service.

Participate/lead and assist with activities of daily living (ADLs) and Invite, Encourage, and Assist (IEA) residents to participate in ISP-indicated activities; engage residents in life skills and life enrichment activities; respond to resident needs with empathy and respect for individual communication patterns.

Maintain an atmosphere of warmth and calm; follow safety regulations; practice good Standard Care Precautions of cleanliness, hygiene, and health; host and engage in daily activities with residents.

Risk Management and General Safety: Ensure compliance with occupational health and safety requirements; practice safety procedures including PPE, fire safety, SDS, and Lockout Tagout; report accidents/incidents and unsafe conditions immediately; keep hazards out of walkways and exits; handle exposure to bloodborne pathogens and hazardous materials according to policy; manage oxygen tanks safely.

Housekeeping and Laundry Services: Maintain and clean resident rooms; manage resident laundry; handle dining room linens and general housekeeping tasks.

Dining Service: Serve meals, promote pleasant dining experiences, assist with setup/cleanup, participate in pre-meal meetings, follow dietary instructions, document changes in dining habits, provide room service as needed; follow safe food handling practices.

Reminiscence Neighborhood Specific: Support life skills activities, attendance at social events, and integration of resident life skills into daily routines; blend multi-sensory experiences into daily activities; engage in monthly letters home with residents and families.

Training and Team Success: Collaborate with team toward goals; demonstrate Team Member Credo and Principles of Service; contribute to engagement programs and participate in surveys; attend meetings and training; ensure infection control and regulatory training compliance; perform other duties as assigned.

Core Competencies: Ability to stay calm in difficult situations; manage multiple priorities; effective written and verbal communication; strong organizational and time management; good judgment and problem solving.

Experience and Qualifications

High School diploma/GED (as required by regulations).

CPR and First Aid as required by regulations.

Must be at least 18 years of age; prior experience with seniors preferred.

Desire to serve and care for seniors; ability to act in the resident’s best interest.

If applicable, drivers of Sunrise vehicles must sign the Driver job description.
