Responsibilities:
- Assess client needs and develop personalized care plans to enhance their well-being.
- Coordinate and oversee the delivery of care services to ensure quality and compliance.
- Communicate regularly with clients, families, and healthcare providers to ensure continuity of care.
- Monitor client progress and adjust care plans as necessary to meet evolving needs.
- Provide training and support to caregivers to ensure high-quality service delivery.
- Maintain accurate documentation and records of client interactions and care plans.
- Participate in team meetings and contribute to a collaborative care environment.
- Stay updated on industry trends and best practices to enhance service offerings.
Requirements:
- Bachelor's degree in Nursing, Social Work, or a related field preferred.
- Minimum of 2 years of experience in care management or related healthcare field.
- Strong understanding of home care regulations and best practices.
- Excellent communication and interpersonal skills for effective client interactions.
- Ability to work independently and prioritize tasks in a fast-paced environment.
- Compassionate demeanor with a commitment to client-centered care.
- Valid driver's license and reliable transportation required.
- Current CPR and First Aid certification is a plus.

Compensation: $25.00 - $30.00 per hour
