What to Expect (Job Responsibilities)

Coordinate care for individuals using assessment, care planning, and evaluation to achieve quality outcomes

Conduct in-depth health risk assessments and develop appropriate care plans

Serve as the main point of contact to ensure services are rendered effectively

Monitor and adjust care plans based on ongoing evaluations and member needs

Collaborate with various stakeholders, including members, caregivers, and healthcare providers, to address care issues

What is Required (Qualifications)

3-5 years of experience in Social Work, Nursing, or a healthcare-related field, or relevant experience in lieu of a degree

Detailed knowledge of cost-effective coordination of care and referral processes

Ability to analyze trends and make decisions based on data interpretation

Strong verbal and written communication skills for effective collaboration

Must possess a valid driver's license

How to Stand Out (Preferred Qualifications)

Associate or Bachelor's degree in a relevant field

Certifications such as Certified Case Manager (CCM) or Licensed Clinical Social Worker (LCSW)

Experience in utilization management, quality assurance, or community health

Knowledge of business management skills, including cost/benefit analysis and negotiation
