Under the direct supervision of the Community Services Unit Director, the Care Coordinator Supervisor will have a variety of responsibilities including the supervision of care coordinators, overall caseload management, review of care plans and client files, development of community resources, and the development of in-service training programs. This position requires extensive knowledge of Shawnee’s Comprehensive Care Coordination services, the aging network and knowledge of other social service agencies in the service area.
The Care Coordinator Supervisor is responsible for training and supervision of Care Coordinators, including assuring that training requirements are met and ensuring compliance with the Rules and Regulations of the Illinois Department on Aging, the Area Agency on Aging, and Shawnee Health.
Represents Shawnee Health in the most positive manner with prospective, former and current patients, clients, their families and the community we serve.

This will include assessing and serving the needs of the underserved/vulnerable populations and designing programs and services, which are culturally and linguistically appropriate.
Handles confidential information with tact and discretion.
Provides ongoing supervision of assigned care coordinators, which includes reviewing files, developing systems and utilizing, monitoring caseload and conducting performance evaluations.
Accompanies care coordinators in the field and works with care coordinators on strengthening areas of weakness or poor understanding.
Develops in-service training programs for care coordinators.
Works within the community to assist care coordinators in the development of community support systems and resources.
Participates in supervisory meetings.
Works with Community Service Unit Director and other management staff to develop new programs, implement program enhancements, and recommend procedural changes to improve the service delivery system. Assist in development and writing Best Practice Protocols for case management services.
Represents the position of the agency in client staffing and special circumstances which involves complex cases requiring administrative decisions and guardianships.
Serves as on-call supervisor.
Coordinates, supervises and trains student interns for specific program assignments and projects.
Identifies gaps in services and reports to Program Director identified needs.
Conducts quality assurance reviews which includes supervising the implementation of client satisfaction surveys.
Conducts public education, presentations and training sessions to the community at large, upon request.
Trains all new case management staff under the direction of the Community Services Unit Director.
Attendance
Employee must maintain regular, punctual attendance consistent with Shawnee Health policies and procedures. Employee must to report to work and be on time for each scheduled shift unless scheduled absent or on approved leave in accordance with federal and/or state law. Regular employee attendance is essential to the efficient operation of Shawnee Health and is an essential component of employee’s job duties.

Compliance with Policies/Procedures
Employee will maintain compliance with Shawnee Health policies, procedures, and on-going education requirements. Employee will maintain compliance with program specific standards and regulations.
Occupational Safety and Health Administration (OSHA) Health & Safety
Employee will comply with all standard operation procedures of Shawnee Health regarding employee health and safety, including but not limited to OSHA regulations (Blood borne Pathogen Standard and Hazardous Communication Rules), Environmental Protection Act (EPA) regulations governing waste disposal, and completion of unusual occurrence reports.
Physical Requirements
Requires lifting and carrying up to 30 lbs.
Position requires navigational ability in client’s homes, offices, and buildings both with and without ADA accessibility.
Position requires frequent standing, bending, and sitting.
Requires normal range of hearing and vision.
Required to wear safety gear provided by Shawnee when entering infested environments or when communicable diseases are present.
Care Coordinator Supervisors are required to wear safety gear provided by Shawnee during snow/ice events. This gear must be worn to enter/exit office buildings, participant’s homes, banks, other locations during activities for Shawnee Alliance/Shawnee Health.
Required Working Conditions
Climate-controlled office environment.
Required to work flexible schedules, including some nights, early mornings and weekends.
Must complete continuing education hours as required by IDOA and EAAA.
Reliable Transportation on the job is mandatory. Have a good driving record to drive company-owned vehicles.
Licenses & Certifications
Must have a valid Illinois driver’s license and auto liability insurance on personal vehicles. Must receive and maintain Illinois Department on Aging eligibility certification. Must receive and maintain SHIP (Senior Health Insurance Program) certification.
Additional Job Duties And Responsibilities
Additional Job duties as assigned.
Represents Shawnee Alliance on various boards, task forces, etc., where a representative who is well versed in aging issues is appropriate.
Serves as an on-call supervisor.
Will be expected to attend out-of-area conferences, trainings, meetings, workshops, etc.
Serves as member of the Shawnee Alliance Management Team.

Education & Experience Requirements
Mandatory – B.S. degree in human service field and/or a R.N.
Preferred – Minimum of two years’ experience working with older adults.
Master's degree in Social Service preferred emphasis in Social Work.
Familiarity with Community and government health and social services is highly desirable.
Competency Requirements
High level of decision making with minimal supervision.
Must complete continuing education hours per year as required by the Illinois Department of Aging and the Egyptian Area Agency on Aging.
Must develop knowledge base of IDOA rules and regulations, community resources for the older adult, and state and federal programs.
Must demonstrate good communication skills.
Must demonstrate an understanding of the life experiences of older adults, and the challenges they face.
Must have networking skills and ability to work with staff at other agencies.
Skilled in mediation/negotiation with knowledge of group dynamics and family dynamics.
