Job Summary / Purpose
The RN Care Coordinator is responsible for overseeing the progression of care and discharge planning for identified patients requiring these services. The role promotes quality of care, cost‑effective outcomes, and follows hospital policies, standards of practice, and Federal and State regulations. The RN Care Coordinator collaborates with utilization management, nursing, physicians, ancillary departments, insurers and post‑acute service providers to progress care toward optimal outcomes. The RN Care Coordinator advocates for patients and families by addressing patient choice, spiritual needs, cultural, language, and socioeconomic barriers to care transitions, while striving to enhance the patient experience. Additional duties include utilization review as necessary.
Responsibilities
Oversee progression of care and discharge planning for identified patients.
Promote quality of care and cost‑effective outcomes, ensuring compliance with hospital policies and regulations.
Coordinate and collaborate with utilization management, nursing, physicians, ancillary departments, insurers, and post‑acute service providers.
Advocate for patients and families, addressing choice, spiritual needs, cultural, language, and socioeconomic barriers to care transitions.
Enhance patient experience throughout care transitions.
Perform utilization review as needed.
Qualifications
Required Education and Experience
Graduate of an accredited school of nursing.
Minimum two (2) years of acute hospital clinical experience or a Master’s degree in Case Management or Nursing field in lieu of one year experience.
Preferred Education And Experience
Bachelor’s degree in Nursing (BSN) or related healthcare field.
At least five (5) years of nursing experience.
Required Licensure and Certifications
WA RN License.
BLS required within three months of hiring.
Preferred Licensure And Certifications
Certified Case Manager (CCM), Accredited Case Manager (ACM‑RN), or UM Certification preferred.

$48.32 – $83.29 per hour
