About the role
This role involves managing client caseloads within assigned service programs. Responsibilities include conducting client intakes, assessing needs, and developing care plans to promote client independence by overcoming barriers. The Care Coordinator builds strong client relationships, coordinates support with other service providers, makes referrals to internal and external resources, facilitates client team meetings, and ensures follow-up to achieve client goals.

Minimum Qualifications
Associate degree in a related field OR 1 year of experience in a client‑direct service setting (DSP, CNA, human services, customer service, or related field)
Valid driver’s license with acceptable driving record, reliable transportation, and proof of insurance
Ability to travel to outer regions as assigned
Required to complete state care‑coordination training after hiring
Successfully pass a criminal background check, drug screening, and obtain ND DHS pre‑employment approval
Complete all required training as defined by Community Options

Compensation
Salary: $19–$22 per hour, depending on experience (DOE).
Physical Demands and Work Environment
Ability to climb stairs, balance, stoop, kneel, crouch, reach above shoulder, bend, twist, sit, stand, walk, push, pull, lift, and grasp; fine motor manipulation; talk; hear; perform repetitive motions; occasional lifting up to 50 pounds
Vision: close, distance, color, peripheral, depth perception, and ability to adjust focus
May be required to perform physical de‑escalation techniques for certain customer behaviors
Work in indoor and/or outdoor settings (office, home, community); travel by motor vehicle as needed
