Job Summary
The Care Coordinator’s primary responsibility is service linkage and care coordination for children, youth, and families with significant behavioral health needs. The role requires a thorough understanding of local communities, strong relationship‑building with community agencies, and strategic identification of supports to collaborate with Child and Family Teams. Care Coordinators ensure families have a voice and choice in all coordinated care and services.
Pay Range
$20.19 - $25.03 per hour, based on experience, education, and/or licensure.
Essential Functions
Join with families to identify care coordination needs and services in line with service delivery standards and program outcomes.
Work with families to define cultural factors that influence strengths, functioning, and family interaction styles to ensure engagement and success in care planning.
Identify strengths of children, youth, and families for utilization in care coordination and healthy outcomes.
Coordinate family‑based services in homes, schools, and communities.
Ensure services identified on care plans are appropriate, least restrictive, and meet safety and treatment needs.
Engage and build positive relationships with children, youth, and families alongside child and family teams.
Develop collaborative and creative partnerships with community resources.
Maintain necessary documentation, participate in program evaluation, attend planning meetings, cross‑systems training, and acquire knowledge of community resources.
Remain current with all training requirements (e.g., High Fidelity Wraparound, MI, Cultural Humility).
Perform additional duties as assigned.
Minimum Requirements
Experience providing services to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or related public sector services.
Three years of experience with a high school diploma or equivalent; or two years with an associate or bachelor’s degree; or one year with a master’s degree or higher.
Lived experience is encouraged.
Knowledge and experience in Hi‑Fidelity Wraparound preferred (certification provided at employment).
Two years of experience in a coordinated supportive services or care coordination role preferred.
Experience working with individuals with autism spectrum disorders and developmental disabilities preferred.
Experience in one or more of the following areas: family systems; community systems and resources; case management; child and family counseling or therapy; child protection; child development.
Cultural humility with training and experience to manage complex cases.
Qualifications to work with children and families experiencing serious emotional disturbance, trauma, and/or co‑occurring behavioral health disorders within child‑serving systems.
Excellent organizational skills with ability to prioritize multiple tasks.
High degree of cultural awareness.
Experience with multi‑need individuals and families.
Broad knowledge of community service systems.
Willingness to participate in and lead cross‑systems care coordination.
Effective verbal and written communication skills.
Proficiency in an Internet‑based, automated office environment.
Valid driver’s license required.
