Work Schedule : 8 a.m. to 5:30 p.m. Monday–Thursday; 8 a.m. to 12:00 p.m. Friday. 8 a.m. to 4:30 p.m., Monday–Thursday (Eudora & Wilmot), Friday 8 a.m.–12:00 p.m. Adjustments in schedule or additional hours may be required to complete duties.

Job Summary The Care Coordinator is part of the care team and, under the supervision of the RN Clinic Manager, coordinates care for patients to providers outside of Mainline Health Systems, Inc. The Care Coordinator is involved in continuous Quality Improvement practices and measures as set by Mainline Health Systems, Inc. and in which the practice focus is based on a Patient Centered Medical Home model of care.

Key Responsibilities Assist patients with referrals to other providers outside of Mainline Health Systems, Inc. and other services.
Make appointments, send referrals to outside providers, complete prior authorization if needed, send Medicaid Referral Form for Medicaid patients, notify patient of appointment, follow-up on appointment, request records, and scan records into EMR. Document all information in the open outgoing referral.
Breast Care: Assist patients in enrollment of Breast Care and entering results into ADH website.
Schedule mammograms.
Interacting with patients and staff on a daily basis.
Communicate with management regarding related issues.
Notify patients regarding missed appointments.
Process requests from patients and staff via electronic messages.
Participate in monthly clinical staff meetings.
Participate in daily huddles and maintain an active role within the care team.
Serve as part of the care team; follow and implement workflows that support and sustain MHSI Patient Centered Medical Home model of care.
Willingness to attend and/or complete continuing education related to this position and/or any other topics as deemed necessary to MHSI.
Complete continual job training processes set forth by Mainline Health Systems, Inc.

Qualifications All listed below are key skills necessary for the position.
High School Diploma or GED required. Bachelor of Social Work or health care related field preferred.
Ability to communicate clearly, take direction, work well with a team, and be self-motivated.
Detail oriented.
Ability to navigate and utilize electronic medical records.
One year of experience in medical field/setting.

Work Environment Office environment in a controlled atmosphere building. This is an OSHA low-risk position.

Reporting Relationships The Care Coordinator reports to and is directly supervised by the Clinic Manager who reports to the Director of Operations. If the Clinic Manager is unavailable due to illness or vacation, the Care Coordinator reports to the Director of Operations or assigned administrative staff.

Evaluation The evaluation of work performance will be ongoing and carried out by the Clinic Manager with consultation of the Director of Operations. The evaluation will include duties, responsibilities, employee attitudes, general working behavior, and willingness to do extra work. Formal evaluations are conducted annually but may be initiated at other times by the RN Clinic Manager, Director of Operations, COO, or provider in charge.

Status This is a FLSA Non-Exempt position. This is an OSHA low-risk position. This is an ADA negotiable position.
