Qualifications
Completion of a Medical Assisting program required
LPN license required or Certified/Registered MA required
Active BLS required
EMR experience preferred
Job Responsibilities
Provides telephonic outreach to patients identified as needing follow-up from ordered referrals to specialty care and diagnostic testing. Patients with orders may have chronic health conditions and need preventative services.
Addresses and coordinates Transitions into Care after a Hospital discharge.
Assist in identifying gaps in care utilizing NextGen and Eagle Dream/Population Analytics. Care Gaps include cervical cancer screenings, colon cancer screenings, breast cancer screenings, well child checks, immunizations, and annual wellness exams.
