Physicians’ Primary Care of Southwest Florida is a premier physician owned and managed primary care practice with locations in Cape Coral, Estero, Fort Myers, and Lehigh Acres. We are currently seeking a Care Coordinator for our Family Practice Division in Fort Myers, FL. This full‑time position is conveniently located in our Administration building off College Parkway and offers a flexible schedule, Monday‑Friday, day shifts.
Sample Of Duties
Work with medical staff to develop, implement, and carry out programs in chronic disease management for patients with diabetes, coronary artery disease, chronic obstructive pulmonary disease, and congestive heart failure.
Review provider schedules and individual patient charts and assist the care team in coordinating care for visits and future healthcare needs using Care Management criteria.
Develop and coordinate patient outreach programs.
Provide an effective communication link between patients and medical staff.
Maintain strict confidentiality and adhere to all HIPAA guidelines and regulations.
Position Requirements
Minimum of three (3) years of clinical experience.
Great attention to detail and the ability to record information accurately.
Ability to develop and maintain a good rapport and a cooperative working relationship with providers, staff, and patients.
Prior experience in care management, quality improvement, medical records reviews, disease/case management preferred.
Graduate of an accredited school of nursing with a valid and active Florida LPN license.
