Job Summary
The Care Coordinator II is responsible for coordinating multiple aspects of the youth’s and/or families’ care. This role provides the coordination and delivery of high‑quality psychosocial care to youth and families. It works with other care providers and community partners to ensure that the overall needs of the client are met. The Care Coordinator II may lead projects or mentor program staff. This full‑time (40 hours/week) position has a flexible schedule and will primarily be a mobile/remote blend. A reliable vehicle is essential, with mileage eligible for generous reimbursement.

Essential Functions

Coordinate multiple aspects of the youth’s and/or families’ care.

Assess the youth and/or family’s needs and strengths while supporting development of the individual service plan in collaboration with the youth and family(ies) and the multi‑disciplinary treatment team.

Implement a strength‑based service plan including community‑based visits/home visits, assessments, identifying and coordinating collateral resources, recognizing gaps, assessing results, and communicating with the multi‑disciplinary treatment team.

Utilize experience, education, and age‑specific knowledge and cultural competence to assess and support youth and/or families to achieve positive outcomes.

Develop, maintain, and update all required documentation in a timely manner utilizing effective written and oral communication.

Program Specific Functions for One Family:

Introduce the One Family program and the Family Group Decision Making (FGDM) model and explain how the program can help families in development of a plan of care for their children and/or vulnerable adults.

Work in close communication with the County Caseworker to advocate for and support the family to ensure the safety and care of the children.

Partner with family, extended family, and kinship caregivers to help identify all needs and safety concerns, as well as the strengths and resources within the family system.

Meet with family to identify resources, potential caregivers and needed service providers.

Coordinate all details related to implementation of a family meeting.

Host family meeting in alignment with Family Group Decision Making (Conferencing) Model.

Qualifications

Bachelor’s degree required.

Minimum 18 months of relevant experience required.

Special Requirements

Unrestricted, valid NYS driver’s license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards.

Pay
$21.40 Minimum pay rate, $31.00 Maximum pay rate, based on experience. This pay rate may include a Regional and/or Department premium.
