Job Summary
The Care Coordinator II is responsible for coordinating multiple aspects of the youth’s and/or families’ care, providing high‑quality psychosocial care, and ensuring that the overall needs of the client(s) are met. The Coordinator works with other care providers and community partners, may lead projects or mentor program staff, and supports a flexible Tuesday‑Saturday full‑time schedule with on‑call responsibilities.

This position will be on the Monroe Region Youth ACT team. The Youth Assertive Community Treatment (Youth ACT) program serves children/youth with serious emotional disturbance who are returning from inpatient or residential settings, or whose needs have not been met in traditional community–based settings. Referrals come through Children’s Single Point of Access (C‑SPOA), a centralized referral system for children with serious emotional disturbance.

Essential Job Functions

Coordinate multiple aspects of the youth’s and/or families’ care.

Assess the youth and/or family’s needs and strengths while supporting the development of the individual service plan in collaboration with the youth, family, and multidisciplinary treatment team.

Implement strength‑based service plans, including community‑based visits, home visits, assessments, and coordination of collateral resources.

Use experience, education, age‑specific knowledge, and cultural competence to assess and support youth and families in achieving positive outcomes.

Coordinate and deliver psychoeducational training for youth and/or families and groups as needed.

Meet with youth individually to assess and teach skill development, and report on progress.

Organize and facilitate skill‑based groups for youth.

Develop, maintain, and update all required documentation in a timely manner using effective written and oral communication.

Provide or arrange transportation for youth and/or families to and from appointments as needed.

May be assigned to lead projects or mentor program staff.

Provide on‑call support for Youth ACT programs.

Perform other duties as required for the role.

Education & Experience

Bachelor’s degree required.

Minimum: 18 months of relevant experience.

Special Requirements

Unrestricted, valid NYS driver’s license for a minimum of 1 year with a clean driving record and sufficient insurance coverage to meet agency standards.

CPR certification required or obtained within 60 days of hire for the Youth ACT program.

Knowledge, Skills & Abilities

Understand and adhere to all federal, state, and local statutes, regulatory agency standards, and Hillside policies.

De‑escalate and manage crisis situations in person and by phone.

Deliver psychoeducational training.

Work collaboratively as part of a multidisciplinary treatment team.

Use independent judgment to address a series of issues.

Be capable of independent, self‑directed work.

Physical Demands & Work Environment

Must be able to work a variable schedule, including evenings and weekends.

Daily physical demands include:

Sitting (up to 6 hours).

Occasional standing (up to 2 hours).

Occasional walking (up to 2.5 hours).

Driving several times a week up to 6 hours.

Continuous balancing (up to 8 hours).

Occasional bending, stooping, climbing, kneeling, pushing, pulling, reaching forwards or down, reaching overhead, running, and twisting (up to 2.5 hours).

Weekly lifting up to 10‑15 lbs. daily.

Manual dexterity required, with frequent use of both hands and fine manipulation skills (approx. 1‑2.5 hrs).

Occasional exposure to dust, fumes, gases, chemicals, or smoke.

Ability to change positions as needed.

$21.40 Minimum pay rate, $31.00 Maximum pay rate, based on experience.
