Overview Join to apply for the Care Coordinator II role at Behavioral Health Network, Inc (BHN).
What You’ll Do Provide high-quality Wraparound services for children/youth and families.
Conduct comprehensive assessments, including CANS, CRAFFT (12+), diagnostic assessments, and Serious Emotional Disturbance determinations.
Convene and facilitate Care Planning Team (CPT) meetings to develop and update Individualized Care Plans (ICP).
Maintain consistent communication with youth, families, and care teams to identify strengths, needs, and effective strategies.
Support youth and family participation in goal setting and implementation.
Identify and connect families with medically necessary services.
Collaborate with state agencies, schools, healthcare providers, and community resources.
Develop transition plans when goals are met.
Obtain insurance authorizations as required by Managed Care Entities.
Maintain service productivity and caseload expectations.
Ensure timely documentation and completion of Risk Management/Safety Plans.

What You’ll Bring Master’s Degree in a Human Services-related field (license-eligible).
CANS-MA certification (required).
Knowledge of mental illness/SED in children/adolescents and treatment techniques.
Familiarity with internet, word processing, and email systems.
Experience navigating child/family-serving systems (preferred).
Knowledge of community resources (preferred).
Experience facilitating meetings or groups (preferred).
Demonstrated cultural competence and ability to work with diverse populations.
Strong organizational, communication, and facilitation skills.
