Care Coordinator - Human Services of Faribault and Martin Counties

Responsibilities
In this position, you will conduct MnCHOICES assessments to understand each individual's strengths, needs, and goals, ensuring they receive personalized care plans tailored to their circumstances. You will work closely with clients and their representatives to develop community support plans, offer guidance on available resources, and provide information about Minnesota Health Care Programs. Additionally, you will oversee ongoing case management for Home and Community-Based Services (HCBS), monitoring and adapting care plans as necessary to ensure quality and effectiveness. Collaboration is at the heart of this role. You will build strong, professional relationships with clients, caregivers, and partner organizations, all while maintaining high ethical standards.
Qualifications
Minimum Qualifications: A bachelor's degree from an accredited four-year college or university with a major in social work, psychology, sociology or closely related field. OR a bachelor's degree from an accredited four-year college or university with a major in any field and one year of experience as a social worker in a public or private social services agency. Bilingual (English/Spanish) highly valued.
