Base Pay Range $55,000 - $60,000
Overview Westchester Jewish Community Services (WJCS), a nonprofit founded in 1943, is the largest provider of licensed outpatient community mental health services in Westchester County. WJCS offers state‑of‑the‑art programs and compassionate care to more than 20,000 people of all ages and backgrounds each year. We are seeking a dedicated High Fidelity Wraparound (HFW) Care Coordinator to make a meaningful difference in the lives of youth, adolescents, and young adults with serious mental health challenges. Training will be provided to become HFW Certified, if not already in possession of such certification.
Responsibilities Develop individualized care plans in collaboration with families and providers.
Serve as the point person for each youth’s care, coordinating services and supports.
Partner with a range of providers and family members to ensure success and positive mental wellness.
Provide crisis intervention and prevention services as needed; develop and implement safety and crisis plans.
Participate in OMH booster and/or re‑certification training requirements.
Maintain fidelity to the HFW process and procedures set forth by OMH.
Maintain accurate and timely documentation.
Requirements Bachelor’s degree in Social Work or related field with at least 2 years of relevant experience, or a Master’s degree with at least 1 year of experience.
Strong ability to engage, support, and empower youth with mental health challenges and/or medical needs.
Excellent communication and collaboration skills.
Valid driver’s license; vehicle required.
Bilingual English/Spanish preferred.
NYS High Fidelity Wraparound Certification, or willingness to participate in training to obtain certification.
