Job Overview:
Join us at Legacy Community Health as we pioneer transformative health care solutions within the school systems of Houston. As a School-Based Health Care Coordinator (Bilingual), you will integrate health and social resources into the academic environment, driving forward student wellness and educational success.
Key Responsibilities:
Engage daily with students, parents, and guardians through phone, email, and face-to-face interactions.
Collaborate with the SBHC team and community organizations to execute a dynamic care plan enhanced by digital health summaries and action plans.
Integrate health information technology for proactive care delivery and continuous monitoring.
Facilitate seamless navigation across health, education, and community systems for students and families.
Leverage data collection and analytics to connect students and families to community support resources efficiently.
Harness cultural and language assets to effectively address student and family needs.
Maintain compliance through meticulous care coordination documentation.
Innovate outreach strategies at school events to enhance service utilization.
Minimum Qualifications:
High School Diploma or equivalent; post‑hire completion of specialized training programs required.
Minimum of two years of experience within health care, schools, or community systems.
Associate's or Bachelor's degree preferred; experience in care navigation or direct patient care is advantageous.
Strong organizational and communication skills with a focus on empathy and cultural humility.
Proficiency in digital health platforms and data documentation.
Experience with diverse populations and a collaborative team mindset.
Bilingual proficiency in English and Spanish is highly valued.
