As a Care Coordinator at Horizon, you will…

Conduct comprehensive member assessments and develop care plans that identify member needs and strengths.

Be responsible for independently managing a caseload of approximately 40 members.

Travel to meet members in their homes or in the community throughout Niagara County.

Connect members to resources and services in the community.

Collaborate with members care team to ensure proper care is being delivered.

Creatively problem solve with members around practical obstacles, in order to meet their needs.

Qualifications
What makes you a great candidate? Here are the job requirements:

Masters degree in a Human Services related field (Psychology, Social Work, Mental Health Counseling, etc) required.

OR Bachelor’s Degree in a Human Services related field (Psychology, Social Work, Mental Health Counseling, etc); OR Credentialed Alcoholism and Substance Abuse Counselor (CASAC) required.

Additionally - at least 6 months of experience providing health home services, case management, care coordination or experience working in behavioral health in another capacity required OR at least 3 months of internship experience with Horizon in our Care Coordination department required.

OR Associates Degree in a Human Services related field required, with at least 4 years of experience providing care coordination/health home services; a clean, valid NYS driver’s license, three years of driving experience, and reliable transportation for local travel.

Excellent verbal and written communication skills.

Ability to work independently while also being part of a large team.

Organizational, time management, and problem-solving skills.

Location
This position is located at 1000 Young Street, Tonawanda, New York, serving the Niagara County community.

Hours
Monday through Friday - no weekend hours. Schedule from 8:00 AM - 5:00 PM, with flexibility to accommodate your schedule.

Compensation
The range of pay for this position is $21.75 - $22.95 per hour, with starting pay determined by experience and education. Eligible for a bonus earning opportunity with an annual max potential of $4,800, based on volume of service visits and cash-equivalent benefits. Compensation is reviewed annually.

Physical demands

Sitting for extended periods, alternating between standing and sitting.

Local travel between worksites.

Mobility involving repetitive wrist, hand, and finger movements.

Handling light duties, occasionally lifting up to 20 pounds.

Exposure to various environmental conditions including low/high temperatures and outdoor elements.
