Position Overview
For this hybrid position, you will provide comprehensive, person‑centered care coordination and advocacy for individuals with intellectual and developmental disabilities. You will collaborate with a multidisciplinary team to ensure all service needs are met, while upholding a strengths‑based, supportive environment.
Social Workers, Care Managers, and Case Managers are encouraged to apply!
Position Responsibilities
Develop a comprehensive, person‑centered life plan with the individual and their circle of support, including all service providers.
Effectively manage a tiered caseload, tailoring services to individual needs.
Complete enrollment and eligibility documentation.
Meet with individuals in homes, physician/provider offices, and other public places to conduct assessments and provide services.
Commit to a respectful, just, and supportive environment for individuals and coworkers, aligning with the company’s commitment to diversity, equity, and inclusion.
Requirements
Bachelor’s degree with 2 years of relevant experience.
Licensed Registered Nurse with 2 years of relevant experience.
Master’s degree with 1 year of relevant experience.
Minimum bachelor’s degree required by OPWDD in NYS.
Relevant experience may include direct care, support coordination, advocacy (e.g., Direct Support Professional, Teacher, Paraprofessional, Counselor, Social Worker, Care Manager).
Valid New York State driver’s license required. If residing in a bordering state, a valid license from that state is acceptable.
