Responsibilities The Care Coordinator, also known as a Care Manager, has an overall responsibility for coordinating and advocating for all aspects of the individual’s care, including, but not limited to, health and behavioral healthcare, community supports, and other services required to meet the needs of the individual. Care Coordinators and Social Workers use a person-centered, holistic approach, where each person being served has their health care and other service providers collaborate to ensure their wants and needs are met. We are in the business of helping people and always using a strengths-based approach.
