Pay Range: $24 - $25/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities

Shifts: Mon-Fri, 9:00 AM-6:00 PM EST

Conduct outbound and inbound calls to engage members, review care/service plans, and provide ongoing education and support

Coordinate care activities by collaborating with healthcare providers, community partners, and caregivers to ensure member needs are met

Serve as the main point of contact for members and providers regarding care-related inquiries and updates

Perform outreach and follow-up calls (50-75 daily) to verify member information and care plan progress

Conduct service assessments and document findings in accordance with regulatory and company standards

Research and verify members’ and providers’ information to ensure data accuracy and communication efficiency

Prepare and submit daily productivity reports

Maintain confidentiality and compliance with all HIPAA and organizational guidelines

Identify additional needs or barriers to care and refer members to appropriate programs or case management teams

Support members by providing education on benefits, available services, and community resources

Key Requirements and Technology Experience

Key Skills; Minimum 2 years of healthcare or managed care experience

Comfortable with high-volume inbound/outbound calls

General technology proficiency (Excel, Outlook, Word, Teams)

Proven ability to manage high-volume outbound/inbound calls in a professional setting

Proficiency with Microsoft Office Suite (Outlook, Word, Excel) and virtual communication tools (Teams)

Strong communication, organizational, and multitasking skills

Ability to work independently in a remote environment with professionalism and discretion

Bilingual (Spanish or Creole)

Knowledge of medical terminology or care coordination processes

HS Diploma or GED
