Location and Scope Based in the service area of Washington, Orange, and Lamoille counties, the Case Manager will work with older persons to remain in their homes through creative connections with state and community resources.

Support older Vermonters with the requirement that the position does not include weekend or on‑call hours.
Coordinate care and public benefits programs.
Qualifications Works well with others and can empathize with their needs.
Comfortable with computers and strong communicator.
Enthusiastic to learn and grow as a professional.
Ability to work independently and as part of a team.
Reliable transportation.
Experience with data entry and a working knowledge of MS Office (Excel, Word, Outlook).
Experience with senior populations, coordinating care, and public benefits programs.
