About the Program
Care Coordination at CHR is an intensive program for children and youth ages 0-18 (or up to age 22 if in school) with emotional/behavioral difficulties and complex service needs and their families. Care Coordinators assist the family through a child‑specific team that develops individual and family‑focused service plans. The coordinators also provide ongoing consultation with the child‑specific team to provide linkage, coordination, follow‑up, and advocacy for the child and family.
About the Position
Duties & Responsibilities
Assists families in identifying strengths, needs, and creating a family vision to inform the plan of care.
Assesses needs of child and family and their capacity and interest in the Wraparound model.
Assists family in developing a crisis/safety plan.
Coordinates and facilitates Wraparound team meetings every 30 days per scope of service, including the development, implementation and monitoring of individualized care plans for youth and family.
Assists families in identifying, recruiting and engaging natural supports.
Provides support and advocacy for Wraparound teams.
Advocates for child and family rights and access to programs and services.
Markets program and provides training and support to local service providers, schools, and parent support groups.
Demonstrates commitment to the Wraparound principles: family voice and choice, team based, natural supports, collaboration, community based, culturally competent, individualized, strengths based, unconditional and outcomes based.
Demonstrates knowledge of services and resources for youth and their families, or ability to quickly learn about available resources in order to provide appropriate referrals.
Participates in assigned community collaboratives monthly.
Additional duties as required.
Qualifications
Education: Bachelor’s Degree in Human Services related field required.
Experience: Minimum of 2 years in a mental health setting.
Licensure/Certification/Registration: Valid driver’s license. CPR/First‑Aid Certification within 6 months of hire.

Compensation
$19.11/hr minimum, commensurate upon experience. Actual rates determined at the time of offer and are based on relevant experience, adherence to internal equity policy. Some positions include program/location differentials and/or sign‑on incentives.
