Person Centered Services Care Coordination Organization is looking for a passionate Social Work Professional to work within a health home model serving people with intellectual and developmental disabilities. The Care Coordinator, also known as a Care Manager, has overall responsibility for coordinating and advocating for all aspects of the individual’s care, including health and behavioral healthcare, community supports, and other services required to meet the needs of the individual.

Position Responsibilities
Develop a comprehensive, person-centered Life Plan with the individual and their circle of support, as well as their entire service provider team.
Effectively manage a tiered caseload, tailoring services to individual needs.
Complete enrollment and eligibility documentation.
Meet with individuals in their homes, physician/provider offices, and other public places to conduct assessments and provide services.

Requirements
Bachelor’s degree with 2 years of relevant experience OR a Licensed Registered Nurse with 2 years relevant experience OR Master’s degree with 1 year of relevant experience.
Minimum of a Bachelor’s degree is required by OPWDD in NYS.
Relevant experience includes providing direct care or support, coordination, and/or advocacy for individuals. Examples: Direct Support Professional, Teacher, Paraprofessional, Counselor, Social Worker, Care Manager, etc.
Valid driver’s license.
Full-time & Part-time options.
Monday – Friday, 8‑hour shift, no nights, hybrid work arrangement.
