Reports to: Program Director

Exempt/Non-Exempt Status: Non-Exempt

Purpose/Function of Position: Serves as a team lead in crisis response, care coordination, case management, and resource navigation for high-risk youth and victims of child exploitation and/or trafficking. The Care Coordinator is responsible for case coordination of all child sex trafficking (CST), human trafficking, and high-risk victim cases in Hunt County.

DUTIES AND RESPONSIBILITIES

Acts as a single point of contact for service referrals for cases involving exploited or high-risk youth.

Coordinates with all participating agencies and service providers to ensure a strong, cohesive, and collaborative approach to child sex trafficking, human trafficking, commercial sexually exploited youth, and high-risk youth (HRY).

Evaluates CAC cases for risk factors indicative of child exploitation and trafficking.

Utilizes evidence-based screening tools to identify youth with clear indicators of exploitation.

Receives calls 24/7 from referring investigative agencies and responds in person, when needed, to the victim's location.

Assists in facilitating Multidisciplinary Team response.

Coordinates and provides survivor-centered care services and advocacy.

Actively maintains contact with the service providers to follow the status of CST/HRY survivors.

Actively coordinates the collection of data and ensures information is entered within the database software.

Actively coordinates, enhances, and supports efforts to build relationships with investigative agencies and identified service providers associated with all cases regarding child sex trafficking/high-risk youth programs.

Facilitates the collaboration and coordination between investigative agencies and service providers.

Assists in the coordination and facilitation of case coordination meetings for CST/HRY cases and all needed follow-up on action plans.

Serves as a backup facilitator to CST/HRY and MDT meetings.

Works closely with Program Director, Sexual Assault Advocates, and CAC Advocates regarding the coordination of comprehensive services and the ongoing status of survivors on the continuum.

Assists the Program Director and Advisory Council in advancing protocols.

Maintains database and tracking system of service providers.

Performs other duties as assigned by the Program Director and other CAC Directors.

QUALIFICATIONS AND SKILLS

Bachelor’s degree in social work or related field from an accredited university required.

Experience with facilitation and coordination of meetings required.

Experience working with child abuse intervention, DFPS, Law Enforcement, and the DA’s office required.

Knowledge of sexual abuse dynamics with CSEC/DMST and/or willingness to receive training required.

Experience in project management preferred.

Knowledge of investigative protocol preferred.

Experience working in the CAC model/case review process preferred.

Existing relationships with HCCAC partners and members of the Hunt County MDT team preferred.

Experience with diverse cultures, languages, and situations preferred.

Advanced level proficiency with MS Office.

Strong communication and interpersonal skills required.

High-priority database management skills, fostering relationships with partners and service providers, and implementing logistics for the Hunt County Care Coordination response and continuum of care for CST and HRY survivors.

Strong organizational skills, ability to work independently, and manage multiple projects and tasks simultaneously.

Candidates should ensure accuracy and maintain strict confidentiality in all work, especially when handling sensitive information.

ESSENTIAL FUNCTIONS

Follow all protocol procedures and make critical decisions regarding the response continuum on a 24/7 basis for CST and HRY cases.

Perform duties seated with occasional standing or walking; occasionally move around the office to access file cabinets and office machinery.

Limited physical effort required, including lifting and carrying objects up to 25 pounds.

Must have access to a vehicle and possess a valid driver’s license with proof of insurance.
