Additionally, we have a network of urgent care centers across the greater New Orleans area, staffed by over 2,800 physicians specializing in a wide range of medical fields.

Essential Functions

Develop individualized care plans with prioritized goals considering patient and caregiver preferences.

Assess patients' health status through outbound calls.

Identify gaps or barriers in treatment plans and coordinate care, including referrals.

Ensure smooth discharge processes with necessary services and medication reconciliation.

Triages symptom-based calls per protocols for various patients.

Enroll patients in home monitoring programs and manage alerts.

Provide general health guidance from approved sources, assessing caller needs.

Document workflows in EPIC system for tracking and quality purposes.

Educate patients on self-management, chronic, and acute disease processes.

Partner with providers to meet quality and cost management goals.

Monitor enrolled members throughout the care continuum.

Engage with leadership and physician groups to discuss clinical cases.

Participate in case conferences to develop tailored care strategies.

Job Qualifications

LA CNA Certification required.

Adult First Aid & CPR Certification required.

Memory care experience preferred.

Experience working with elderly preferred.

Work Shift
Variable hours, United States based.
