How you make a difference The Care Coordinator is a key member of a collaborative care team that provides treatment services to patients with mental health and behavioral disorders. They work closely with the patient's psychiatrist, Competency Intervention Specialist, and mental health providers to develop and implement treatment plans that are tailored to the patient's needs. The role involves providing case management services, crisis intervention, counseling, and skills training to promote effective psychiatric rehabilitation.
Key Responsibilities Collaborate closely with the patient's treatment team to coordinate mental health care.
Monitor patients for changes in symptoms and treatment side effects and provide brief interventions using evidence-based techniques.
Facilitate patient engagement in mental health care and assist in implementing treatment plan changes for patients not improving as expected.
Responsible for coordinating activities related to trainings, trainers, and trainees and integrating PERT principles into the milieu.
Apply sensitivity to cultural diversity issues, critical thinking, and sound judgment to different situations while paying close attention to details and working under stress.
Qualifications & Requirements Education
Bachelor’s degree in behavioral sciences, social work, or psychology; Bachelor’s degree in a non-behavioral health field of study with significant coursework in behavioral science plus a case management certificate from a certified and accredited program with a professional school of behavioral science; May substitute a Master’s degree in social work, substance abuse, education, counseling, psychology or other related field if no prior experience in mental health.
Experience
Minimum of one (1) year of related work experience.
Knowledge of the DSM, the chronically mentally ill, and community resources.
Must be able to communicate effectively with all levels within the facility, in addition to a variety of outside customers with varying interests.
Licenses/Certifications
Will require application as an Agency Affiliated Counselor within seven (7) days of employment.
