Care Coordinator Position at Suncoast Center, Inc.
Minimum Qualifications
Education: Bachelor’s degree from an accredited college or university in any field.
Licensure: Not required.
Experience: One year of related case management/care coordination service to adults or children. Must be able to assist clients struggling with multiple stressors. Understanding of mental illness and community resources required.
Additional Requirements: Completion of AHCA approved targeted case management training within 3 months of hire; certification through the Florida Certification Board within 6 months of hire; age 21 years or older; negative drug screening before hire and throughout employment; clear criminal background check before hire and annually; valid Florida driver’s license and vehicle insurance with acceptable driving record.
Working Conditions: Services provided on-site in the office. Schedule may be irregular based on client availability.
Computer Literate.
Job Duties
Meet with clients to accurately assess and document symptoms, diagnosis, and interventions; ensure all program criteria are met for opening the case.
Complete all documentation to open and close clients in the Care Coordination Program.
Maintain continuity of care per Florida statutes and administrative codes.
Within 30 days link clients with necessary resources and follow up as applicable.
Use appropriate crisis intervention skills and anticipate/prevent crisis.
Provide services utilizing principles of recovery, treating each client as a partner.
Stay current on psychotropic medication, side effects, and DSM diagnoses; recognize when medical services are appropriate.
Complete treatment plan reviews, progress notes, and other documentation accurately and timely.
Assist medical providers with completing applicable forms.
Link individuals/families with necessary resources and monitor effectiveness.
Provide culturally competent, solution-focused, strength-based, recovery-oriented care coordination services as needed.
Meet productivity standards set by Suncoast Center.
Other duties as assigned.
