Care Coordinator – Peninsula Community Health Services (Washington)

Pay Range
Base pay: $16.66/hr - $19.39/hr.
Job Responsibilities
Act as a liaison among patients, caregivers, providers, and community resources to ensure access to clinical services, including medical, behavioral health, dental, and pharmacy care.
Help patients connect with transportation resources and provide transportation services.
Act as liaison to secure social services such as housing, food assistance, and employment.
Maintain knowledge of community resources appropriate to patients’ needs.
Provide care coordination and ongoing follow‑up, assisting with transitions of care between hospital, community, and outpatient settings.
Support insurance navigation, including Medicare and Medicaid enrollment, and assist patients aging out of parent coverage.
Use electronic health record systems (e.g., Athena and NextGen).
Other duties as assigned.
Supervisory Responsibilities
This position has no direct reports.
Qualifications
Valid driver’s license (required).
Minimum 1 year of customer service experience (preferred).
Will obtain SHIBA and Medicaid certification if qualified.

Work Environment and Physical Demands
Work involves moving around the facility, reaching with hands and arms, climbing or balancing, stooping, kneeling, crouching, or crawling. Frequently required to lift and/or move up to 10 pounds and occasionally up to 25 pounds. Employees may be exposed to mechanical parts and moderate noise levels.
