Overview Care Coordinator 2 role at Oregon Health & Science University. This clinical role supports nurse coordinators and physicians within the Knight Cancer Institute by performing a variety of clinical administrative and technical functions to ensure seamless care across the care continuum.
Base pay range $28.25/hr - $38.19/hr
Department overview This is a clinical role that supports the work of nurse coordinators, physicians, and Advanced Practice Providers within the Knight Cancer Institute. The Care Coordinator coordinates non-nurse patient caseloads, providing clinical support with order entry, coordination of appointments (pre-transplant and post-transplant), and procedures for a specialized patient population. The incumbent reviews patient records with the provider to coordinate patient needs during the clinic visit and treatment episode. They take direction from nurse coordinators and physicians, using independent judgment, and build and maintain relationships with internal departments that support Knight Cancer Institute patients.
Function/Duties Key Responsibilities & Performance Standards Patient management: coordinate and manage non-nurse aspects of the patient’s clinical care process. Review the plan of care, support in clinic with order entry, AVS preparation, medication refills, patient and family education, place lab and scheduling orders, and communicate appointment times. Monitor Epic inbasket and handle messages within certification scope. Manage refill requests. Educate and document glucose meter use and pulse reading. Perform authorizations and refill requests for Celgene products. May administer post-transplant immunizations as needed.
Care coordination: ensure patients follow through with outside referrals, coordinate hospital admission and cell collection for bone marrow transplant patients and donors.
Communication: provide program information and education to patients, families, and referring offices. Participate in multidisciplinary conferences, relay patient status to support decision making, act as a communication link with physicians and teams. Respond to voice mail and email with up to two-day turnaround where possible. Attend MD/department team meetings. Triage and respond to patient questions in MyChart, phone, email, or fax within scope or escalate as needed.
Documentation/Data management: document clearly and completely, use checklists and sign-out procedures, participate in database input, monitor active department tracking tools, and support protocol implementation and tracking tools.
Program coordination: arrange satellite clinics, coordinate with teams to ensure results and orders are updated in EPIC, attend weekly huddles to review plan of care, and coordinate ancillary services and procedures with patients.

Required qualifications Three years of experience in a hospital or hospital-based clinic setting
High School diploma or equivalent
Current BLS certification at hire
One of the following four Completion of a nationally recognized accredited medical assistant training program with practicum of at least 160 hours
Successful completion of a formal medical services training program of the United States Armed Forces
Current Oregon EMT license (basic or advanced) with NREMT registration
Current Oregon Practical Nurse License
For dental hires: Dental Assistant training with DANB exam
Note: EMT/LPN/Dental Assistant qualifiers may be exempt from some requirements
MA certification details: Certifications must be maintained for duration of employment. Certifications include CMA (AAMA), RMA (AMT), NCMA (NCT), and CCMA (NHC). Competencies may be demonstrated prior to completing probation or internal evaluation period.
Additional requirements: Ability to work independently and as part of a team, knowledge of patient flow, chart scrubbing, panel outreach, data interpretation, EPIC proficiency, Microsoft Office (Excel, Word, PowerPoint), time management, medical terminology, and strong communication and customer service skills. Sensitivity to the needs of Special Needs patients and families.

Experience with Epic and Cadence (scheduling module)
Additional details Fast-paced environment with interruptions and competing priorities. Works with many departments and external customers. Five-day work week; 8 hours per day.
