The Care Coordinator is an integral part of the treatment team for adults who receive services at the Adult Behavioral Health Hospital. The Care Coordinator provides support to clients’ individual treatment plans while in the facility and provides the client with recommendations and referrals upon discharge such as to community supports and resources. The position is responsible for coordination of care with the client and others involved in their care.
Responsibilities
Provides guidance to unit paraprofessional staff (Behavioral Health Professionals) relative to provision of services in treatment plans
Establishes and maintains regular communication with justice system partners and other partners essential to the treatment plan of clients on the unit
Acts as the program’s coordinator of services to ensure continuity of care for each client
Assists with timely completion of psychosocial assessments, Safety and Crisis Plans, and recommendations included in Discharge Summaries
Participates in care coordination/Discharge Planning based on client need
Makes appropriate and timely referrals to other agencies based on client need and services required
Provides support for individuals, families, and groups
Assists in the completion of patient aftercare forms and serves as a client advocate
Maintains thorough and timely documentation in client records
Makes resource and support referrals to individuals in the facility
Assures that client care provided in the department is completed in a respectful manner that upholds the dignity and rights for adult clients
Communicates effectively in writing and verbally with internal and external partners
Performs social counseling services for individuals, families, and groups
Provides cross-coverage in other Behavioral Health Service programs as-needed to support the Department as a whole
Qualifications
Bachelor’s degree in social work or related field earned from an accredited college or university academic program
More than one year in a human services role and one year related to discharge planning or resource coordination for similar populations
Experience working with people with mental health and substance use diagnoses
Driving Requirements
Valid Wisconsin driver's license and appropriate auto insurance coverage as required by North Central Health Care
Must have the use of a personal vehicle
Preferred Qualifications
Current Wisconsin Substance Abuse Counselor or Substance Abuse Counselor In-Training certification
Compensation and Benefits
The pay range for this role is $26.50 to $29.00, based on education and experience
