Responsibilities Provide comprehensive Integrated Assessment and Treatment Plan review and updates (IM+CANS) to community‑based clients in the CCSO service area who qualify for intensive services per State Decision Support Criteria.
Deliver bilingual (Spanish) stabilization and support services to clients and parents, including case management, benefits assistance, and strengths‑based approach.
Coordinate and consult with MCOs, providers, child‑serving systems, and other supports to transition children from institutional or out‑of‑state settings to community‐based living arrangements.
Create treatment plans reflecting client and family preferences and demonstrating medical necessity.
Coordinate Child and Family Team (CFT) meetings and participated in their development, implementation, and monitoring.
Reassess and review treatment progress with clients and families at prescribed intervals.
Participate in MCR screenings whenever possible and create crisis prevention plans.
Attend and complete all required department trainings in a timely manner.
Assist in establishing effective communication between KYC, other agencies, and the community.
Participate in staffing during hospitalizations; plan post‑hospitalization services including discharge planning and scheduling follow‑up appointments.
Maintain core CCS requirements: initial outreach within 7 days after referral; initial Crisis Prevention and Safety Plan within 10 days after enrollment; initial strengths, needs, and cultural discovery within 21 days; CFT meeting every 60 days or 48 hours after MCR event; IM+CANS review every 60 days; CPSP review every 60 days; in‑person contact 1x/month; telephonic contact 3x/month.
Participate in consultation with other agency professionals or paid consultants as needed.
Attend team meetings, regular performance evaluations, and clinical supervision with supervisor.
Qualifications Bachelor’s or Master’s degree in Social Work, Psychology, Counseling, or a closely related discipline from an accredited institution.
Minimum of 2 years experience providing clinical treatment or care coordination with children and adolescents.
Valid driver’s license, proof of insurance, and reliable transportation.
Computer skills, including Microsoft Word & Excel.
Full‑time, exempt position (37.5 hour work week). Must work a flexible schedule, including evening hours 2–3 times per week.
Proficient in Spanish (both reading and speaking) to provide bilingual services.
