Bilingual Care Manager Russian/Ukrainian RN/BSN Work from Home Full-Time Care Managers working from home office - Must be an RN or BSN. Pay is based on experience and productivity. Job Type: Full-time. Job Hours are from 8:30 to 5:00 pm MST.
Main role is to complete patient‑centric comprehensive care plans based on all chronic conditions for Medicare and Medicaid patients with multiple chronic conditions. Bilingual speakers of Russian or Ukrainian are required. The Care Manager must have excellent phone skills, be comfortable calling patients to discuss care, make appointments, provide education, navigate the EHR system, use Microsoft Office (Excel, Word, PowerPoint) and manage time efficiently. Strong communication skills, initiative and self‑motivation are essential. Job duties may expand as needs arise. All services are to be consistent with applicable Medicare regulations for billing CCM services.
