Description of the role:
The BHU LVN case manager at Mission Community Hospital in Panorama City, CA, plays a vital role in ensuring high-quality care for patients in need of behavioral health services. This position requires a skilled and empathetic individual who can provide compassionate support to patients throughout their treatment journey.
Responsibilities:
Collaborate with interdisciplinary teams to create and implement individualized care plans for patients
Monitor and evaluate patient progress, updating care plans as needed
Provide direct patient care, including medication administration and patient education
Coordinate with external agencies and resources to support patients' ongoing needs
Conduct assessments and screenings to identify patient needs and risks
Requirements:
Active LVN license in California
Experience in behavioral health or mental health settings
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced healthcare environment
