Department: MED‑Medical Management (Case Management)

Schedule: Monday through Friday, 8 AM to 5 PM Central

Location: Fully remote, serving members in Illinois (requires Illinois RN license)

Position Purpose
Develops, assesses, and facilitates complex care management activities for physical needs members to provide high quality, cost‑effective healthcare outcomes, including personalized care plans and education for members and their families.

Key Responsibilities

Evaluate member needs, barriers to care, social determinants of health, and recommend and facilitate appropriate care plans.

Develop ongoing care plans, coordinate with providers, specialists, and community resources.

Identify problems or barriers to care and provide care management interventions.

Coordinate with member, caregivers, and care provider team to ensure person‑centered care.

Monitor member status and progress toward care plan goals; update plans as necessary.

Provide resource support for local services (employment, housing, independent living, justice, foster care).

Facilitate care management and collaborate with providers to ensure timely access to needed services.

Perform telephonic, digital, home or other outreach to assess member needs.

Collect, document, and maintain all member information and care management activities.

Educate members and caregivers on disease processes, care gaps, provider instructions, and benefits.

Provide feedback to leadership on opportunities to improve care and quality delivery.

Perform other duties as assigned.

Education & Experience
Requires a degree from an accredited School of Nursing or a Bachelor’s degree in Nursing and 2–4 years of related experience.

License/Certification

Registered Nurse (RN) – state licensure or compact state licensure.

For YouthCare Illinois plan only: Bachelor’s degree and IL RN licensure (must reside in IL).

For Sunshine Health (FL): Minimum two years of pediatric experience; up to 80% local travel may be required.

Pay Range
$55,100.00 – $99,000.00 per year.
