Position Summary / Career Interest The Behavioral Health Case Manager is responsible for providing care/services safely and efficiently for a full range of services to patients of all ages and their families. The primary role is to collaborate, communicate, and facilitate coordination of services during and post-hospitalization as established by the behavioral health care team and executed by the case manager.
Responsibilities And Essential Job Functions Perform the professional, clinical, and/or technical competencies of the assigned unit or department.
Accept responsibility and accountability for achieving optimal outcomes within their scope of practice. Follow policies, procedures, and standards. Assume responsibility for risk and safety issues. Take call as required by department expectations. Perform job-specific responsibilities and demonstrate accountability.
Maintain knowledge and competence related to their position's expectations and practices within their scope. Bring ideas and concerns to supervisor, participate in department decision-making. Maintain current licensure.
Initiate and participate in family conferences to determine psychosocial and discharge planning needs.
Complete psychosocial assessments of patient/family situations, including social, psychological, emotional, financial, and other factors, to facilitate return to the community.
Utilize social work assessments and input from team members to formulate realistic recommendations and discharge plans.
Locate community resources and connect patients with services to assist in their living situations.
Assist with financial concerns, insurance, and applications for social security benefits, Medicaid, Medicare, and understanding State Waivers.
Connect patients with community mental health, psychiatry, psychotherapy, expressive arts therapy, outpatient case management, substance abuse treatment, and support groups.
Coordinate community placements for post-hospital discharge, such as group homes or nursing facilities, based on patient needs.
Advocate for patients and caregivers to access services, ensuring patient rights and support for patient-centered care.
Document all relevant information in the medical record, including initial contact, assessments, daily contacts, conferences, and discharge planning, within 24 hours.
Work with the treatment team to ensure timely discharge and utilize resources for third-party payors.
Participate in case management activities, interdisciplinary meetings, and provide coverage as needed.
Perform other duties as assigned by the Clinical Manager.
Required Education And Experience Master's in Social Work
Required Licensure And Certification Master’s level licensure with the Behavioral Sciences Regulatory Board as a Licensed Master Social Worker (KS), Licensed Professional Counselor (KS), or Licensed Marriage and Family Therapist (KS). State of Kansas Social Work license.
Knowledge Requirements Proficiency in reading and documenting in electronic medical records.
