Job Summary
The Case Manager is a Behavioral Health Technician (BHT), under the direct supervision of a licensed behavioral health professional (BHP). The role provides clinical services that target treatment outcomes and promote the biopsychosocial health and wellness of the client. The Case Manager is a supportive role that delivers services to improve treatment outcomes and meet patient treatment plan goals.

Duties/Responsibilities

Assistance in identifying, implementing, maintaining, monitoring, and/or modifying behavioral health services on a routine basis

Assistance in identifying, finding, and starting/implementing necessary resources that support activities of daily living and/or respond to the needs of other Social Determinants of Health (SDOH) other than behavioral health services

Coordination of care with the patient, their Health Care Decision Maker (HCDM), healthcare providers, family of origin and/or choice, community resources, and other natural support systems and/or people including educational, social, judicial, community, and other State agencies

Coordination of care activities related to ensuring continuity of care between levels of care (e.g., inpatient to outpatient care) and across multiple services (e.g., personal care services, housing services, nursing services, and family counseling) and providers

Assisting patients in applying for social security benefits

Outreach and follow‑up to patients after report of a crisis event

Participation and engagement in formal and informal case staffing, case conferences, and other meetings with or without the patient/HCDM, DR, or their family participating

Supports, collaborates, and manages with assigned clinicians/providers on caseloads and identified needs

Assistance in maintaining, monitoring and modifying covered services as outlined in the patient’s service plan to address an identified clinical need

Other activities, as needed, that address and/or support the patient with identified treatment needs

Supports rescheduling, outreach, and reengagement efforts as needed

Adheres to all rules and regulations regarding patient confidentiality and patient rights as outlined in contracts, NOAH policy, and state and federal regulations

Maintains position productivity expectations

Performs other related duties as assigned

Required Skills/Knowledge/Abilities

Knowledge of case management principles and practices

Ability to establish and maintain effective working relationships

Excellent verbal and written communication skills and ability to interact with individuals of various social, cultural, and educational backgrounds

Ability to understand and follow written and verbal instructions

Excellent organizational skills and attention to detail

Ability to exercise confidentiality, discretion, and good judgment

Proficient with Microsoft Office Suite or related software

Ability to read, write and speak English; Spanish in addition preferred

Education and Experience

High school diploma or high school equivalency diploma and a combination of education in a field related to behavioral health and full‑time behavioral health work experience totaling at least two years

Associate's degree and at least two years of full‑time behavioral health work experience

Bachelor’s or master’s degree plus one year behavioral health experience OR

Bachelor’s or master’s degree with a behavioral health focus

Other Requirements

New hires are required to pass a pre‑employment background check and drug testing (effective 11/1/2022)

Current Level 1 Fingerprint Clearance Card or must pass fingerprint clearance within 7 days following hire
